If your appointment is related to a Worker’s Compensation Claim, please advise us in advance and bring with you the insurer’s details, your claim number, and the name and contact details of your case manager as well as written approval of the coverage of the costs of your appointment by the insurer. If there is no written approval for the cost of your consultation, you will be required to pay for the appointment on the day and then take the receipt to your insurer to claim.
